Generate Quebec TP-64.3-V Forms

If you have Quebec employees who are salaried or earn commissions that claim employment expenses, you’ll want to complete a TP-64.3-V.

Generate Quebec TP-64.3-V Form creates a fillable or non-fillable TP-64.3-V PDF for your employees. For more information on eligible employment expenses, go to Quebec’s Guide.

Additional Information
TP-64.3-V forms aren’t displayed on ASSP or sent via email tax slips. Please use an alternative method to provide these to your employees.

An English TP-64.3-V is created for employees with English selected as their Language on the Personal section of the Employee Profile; a French form is created when French is selected as the Language.

Additional Information
TP-64.3-V forms can be generated for all employees, regardless of whether or not they live in Quebec.

As a Regular User, the following tasks can be completed using the Desktop Application.

How to Complete The Form Defaults

Step 1: Open Generate TP-64.3-V Form.
By default, this can be found in Tax Slip Processing.

Step 2: Select the location to save the form in the Output Folder.

Step 3: Select Include Employee Name in File Name.

  • If selected, the TP-64.3-V will be Tax Year-Form Name- Employee Last Name First Name Initial-Employee Number.PDF.

  • If deselected, the TP-64.3-V will be Tax Year-Form Name-Employee Number.PDF.

Step 4: If you want to make changes to your TP-64.3-V after they’re saved, select Generate as a Fillable PDF.

How to Complete the Employer Declaration

A signature is required on your TP-64.3-V. If you’d like to include an electronic signature, please upload an image and save it on your computer. The image must have a transparent background.

Step 1: Select 2023 in Tax Year if it’s not selected.

Step 2: Ensure the following are correct and update if inaccurate:

  • Employer Name

  • Employer Address

  • ID Number: Your Quebec Employer ID.

  • TQ: Your 4-digit QST extension number.

  • Authorized Person Name

  • Phone Number

  • Phone Number Extension

  • Postal Code

Step 3: Adjust the form’s issue Date, if necessary.

Step 4: Select the arrow if you’d like to add or change the image of your signature that populates the TP-64.3-V Forms.

Step 5: Navigate to the location where you saved your signature and select the file.

Additional Information 
If you don’t see the signature file, ensure you have the correct file type selected.

Step 6: Select Open.

Additional Information
The signature displays so you can preview how the image will appear on the form.

How to Restrict Which Employees Get a TP-64.3-V

You can create employee filters to restrict which employees will get a TP-64.3-V. Only employees who match a selection in each filter will have a TP-64.3-V generated.

You can also create filters based on any of the Employee Profile selections. For more information, skip to How To Use Data Filters. To create a TP-64.3-V for all your employees, skip to the How To Complete Section 3 Of Your TP-64.3-V section below.

Additional Information
These filters only consider your employees’ current information; historical data isn’t considered.

Step 1: Select the Employee Filter from one of the following options:

  • Employee: Filter by selecting the employees. Only employees that match the other filters you’ve created are available for selection here.

  • Positions: Filter employees by their primary position.

  • Locations: Filter employees by their location.

  • Work Group: Filter employees by their work group.

  • Pay Group: Filter employees by their pay group.

  • Job Status: Filter employees by their job status.

  • Additional Employee Fields: Filter employees by the additional fields defined on the Profile Field Names tab in Payroll Parameters

  • G/L Segments: Filter employees by their G/L Account segments.

    • To include employees that Group selected for their G/L Option, select the blank segment.

    • To exclude employees that Group selected for their G/L Option, deselect the blank segment.

Step 2: Select all the options that employees can match to have their TP-64.3-V created.

Additional Information
TP-64.3-V forms are only created for employees with one of the selected options.

Step 3: Repeat Steps 1 and 2 for each employee filter you’d like to create.

Additional Information
If you create multiple filters, employees must match a selected option for each filter to generate their TP-64.3-V.

How to Use Data Filters to Restrict Which Employees Get a TP-64.3-V

You can create data filters to prevent TP-64.3-V forms from being created for some of your employees. Only employees who meet the filter criteria will have a TP-64.3-V created.

A TP-64.3-V will be created if:

  • They meet the employee filters

  • They meet the filter criteria

To create a TP-64.3-V for all your employees, skip to the How to Complete Section 3 on Your TP-64.3-V section below.

Step 1: Select the Data Filter tab.

Step 2: Select Define.

Step 3: Select Insert.

Step 4: Select the field that determines which employees’ tax slips will be created.

Step 5: Select the limiting circumstance of the filter for the condition from the following options:

<                    Less than
>                    Greater than
=                    Equal or exactly as
<=                 Less than or equal to
>= Greater than or equal to
<>                  Not equal to
Like               Includes employees that match the value, when the value has wildcards (*)
Not Like        Excludes employees that match the value, when the value has wildcards (*)
In                    Includes any employees matching one of the values in the list
Not In            Excludes any employees matching one of the values in the list

Step 6: Select the value from the list of available options. The employee must match the value based on the conditions to have a TP-64.3-V created.

Additional Information
If the condition is ‘Like’ or ‘Not Like’, characters in the value can be replaced with wild cards (*). The condition will be met if all the characters, except wild cards, match the value.

Step 7: Specify how multiple filter lines relate to the next filter line by selecting either And or Or in Conjunction.

And        Filtered results meet all criteria defined.
Or           Filtered results meet one or the other criteria defined.

Step 8: Select OK to save the filter.

Step 9: Repeat Steps 3-8 until the filter removes all the employees that shouldn’t have a TP-64.3-V created.

Step 10: Select Exit.

How to Complete Section 3 on Your TP-64.3-V

Section 3 will be completed differently, depending on which sections apply to your employees. The section on the TP-64.3-V will correspond to an Avanti tab.

For example, section 3.2 on the TP-64.3-V corresponds to the 3.2 A tab in Avanti.

Review the questions on each TP-64.3-V section to confirm whether they apply to your group of employees.

  • Yes/No questions

  • Selections for earning and deduction/benefit codes

  • Areas for you to enter additional details

In this section, we are completing a TP-64.3-V for a travelling technician that uses their personal automobile and receives a per-kilometre allowance.

Step 1: Select the 3.2 A tab.

Step 2: Select Yes for Did the employee receive a per-kilometre allowance.

Step 3: Click Select.

Step 4: Select the correct earning code and then select OK.

This employee doesn’t qualify for any other expenses reported on the TP-64.3-V, so we’ve completed Part 3.2 A.

How to Create the TP-64.3-V

Once you’ve made your selections, you can create your employees’ TP-64.3-V.

Step 1: Select Generate.

Important Information
The tax slips will immediately be created once you select Generate.

Step 2: Once your tax slips finish saving, select Print Results to keep a copy for reference. From here, you can either print or save it as a file.

Step 3: Review the PDFs for your TP-64.3-V, make any additional changes required, and then distribute them to your employees.

TP-64.3-V forms are not displayed on ASSP or sent via email tax slips. Please use an alternative method to provide these to your employees.

Step 4: If other employees have expenses to report on a TP-64.3-V, go to the How To Complete The Form Defaults section above and create the additional TP-64.3-V forms.

Additional Information
Each employee should only receive one TP-64.3-V per business number.
